No. A referral is not required unless you have a Department of Veteran’s Affairs Gold/White card or you have a Chronic Disease Management (CDM) plan from your GP.

Depending on the nature of your presenting complaint, you may need to bring along some items to the consultation.

  • Current/Previous orthotics
  • Any X-rays, ultrasounds, CT and MRI reports
  • Your regular footwear – work shoes, sports shoes and everyday shoes
  • Wear appropriate clothing for foot and lower limb assessment
  • Bring you referral IF you have one
  • Your Private Health Insurance card

We require full payment at the time of consultation. We accept HICAPS, VISA, MASTERCARD and AMEX.

Yes. Bring your Health Insurance Card with you.

If you have a Chronic Disease Management plan (CDM) from your GP you can claim a rebate from Medicare. Speak to your GP to see if you are eligible for this plan before coming in for your appointment.

We kindly ask that you arrive 5 minutes early for your initial consultation.

We ask that you provide 24 hours’ notice to cancel or reschedule an appointment.

Melbourne Foot Clinic will prepare a confidential medical record for you. It will contain your name, address, contact details and other information. This record may include information regarding the condition for which you are presenting to this practice for treatment, past medical conditions, Covid vaccination status, as well as both past and current treatments.

Every consultation you attend in this practice requires new information to be added to your medical record. This information may be shared with other health professionals who are actively involved in your health care.
